When members are looking to roll funds out of Simple Choice Super and into their own Self Managed Super Fund (SMSF).

Actions:

1. Request from the member in hard-copy:

     a. SMSF Rollover Form: SMSF Rollover Form

     b. Certified copy of ID

     c. Certified copy of SMSF Bank Statement

2. Forward relevant forms to OVSS via servicemail

Notes:

Ensure ID and Bank statement are both certified and signed off as true copies of the original.
